Declaraction

I/We HEREBY confirm and declare that all whatsoever information details provided and disclosed in and for the purpose of the application form submitted and signed by me hereto are, to my belief and knowledge, true ,accurate and complete.

By signing this Form, I unequivocally and unreservedly AGREE, CONSENT and/or otherwise AUTHORISE BEN LEE AUTOMOBILE, BEN LEE SERVICES & BEN LEE AUTOMOBILE SDN BHD. and/or its authorised representative(s) to verify and/or procure, obtain all necessary and relevant information from any legitimate and reliable sources, including but not limited to, credit reference agencies, financial institutions, public record department or agencies, National Registration Department, Inland Revenue Board and/or any other governmental agencies/departments deemed appropriate by BEN LEE AUTOMOBILE, BEN LEE SERVICES & BEN LEE AUTOMOBILE SDN BHD. for and in furtherance of the purpose stated herein.

I/we further hereby AGREE and DECLARE that BEN LEE AUTOMOBILE, BEN LEE SERVICES & BEN LEE AUTOMOBILE SDN BHD. and/or its authorised representative(s) or affiliated entity(ies) may collect, obtain, store and process my/our personal data that I/we provide or disclose in this form for the purpose and intention related and/or incidental to the legal transaction(s) for which the submission of this Form is required.

In connection herewith, I/we HEREBY give my/our CONSENT to BEN LEE AUTOMOBILE, BEN LEE SERVICES & BEN LEE AUTOMOBILE SDN BHD., to:-

Access personal information to establish or verify my identity and background such as my/our full name, (valid) passport or identity card number, nationality and religion;

Collect, store, retain and process my/our personal data;

Obtain or procure information on payment and credit standing/rating, including but not limited to debit and credit card information, card-holder’s name, expiry date thereof, billing address(es) and related bank account details;

Provide, share and disclose my/our Personal Data to any relevant or appropriate governmental authority(ies), agency(ies) or third party(ies) where required by law, Court Orders, standard legal or official requirements for lawful/legal purposes;

Permit recordings of any call(s) placed by me/us to your customer services;

Retain, store, use as reference any Resume/CVs submitted to us for purposes stated herein.

By way of extension thereto, I/we also agree and consent that my/our Personal Data may be transferred to or copied by or reproduced for any company or legal establishments incorporated within BEN LEE AUTOMOBILE, BEN LEE SERVICES & BEN LEE AUTOMOBILE SDN BHD. which may include/involve exchange or transfer of my/our data to any other location within Malaysia for purposes of submission of details or information for up-dating or verification of such personal data, all of which purposes you shall have access thereto or otherwise to which I may have reasonable access for any corrections to be made thereto.

For avoidance of doubt, Personal Data include all data defined within the Personal Data Protection Act, 2010.
